Several members of the PA REACH team attended the ONC Million Hearts® Conference held in Washington, D.C. on August 14 and 15, 2013.
The conference was developed to provide Regional Extension Centers (RECs) from across the U.S. with additional information and resources about the Million Hearts® initiative, and how health IT can serve as a valuable tool to advance the key aims of this initiative.
The conference also highlighted how RECs can utilize the PDSA cycle through using established Million Hearts® tools to achieve clinical outcome transformation in cardiovascular disease, hypertension and other clinical measures of interest.
During this two-day meeting, ONC and its Federal partners showcased individual practices that are utilizing Health IT to support the Million Hearts® Initiative and other clinical transformation activities as a platform for improving quality of care, and discuss EHR integration with quality initiatives. Theme-focused breakout sessions explored implementation of tools and processes to support quality of care.
Each Regional Extension Center that is working to leverage Health IT to support the Million Hearts® Initiative was invited to create a poster to explain what they are doing and to showcase how they are working with providers to have an impact. This includes work related to improving performance on Million Hearts® related goals (e.g., blood pressure control, cholesterol management, smoking cessation, stroke prevention, and any work related to CAD or CVD).
