Welcome to the Quality Insight PA DOH Project

heart and stethescope.jpgAs you may be aware, the Pennsylvania Department of Health (PA DOH) has been awarded a grant from the Centers for Disease Control and Prevention (CDC) to prevent and control diabetes, heart disease, obesity and associated risk factors. Quality Insights of Pennsylvania has been awarded the work under this grant. We want to thank you for your participation in this important work. Your practice is one of nearly 150 who have committed to this project to date and we hope to recruit more across Pennsylvania.

Quality Insights has created this website as a central location to provide information to your team members, in addition to our e-Bulletins.

Our goals and activities for this project will be to:
  • Collect monthly NQF measure 0018 - Controlling High Blood Pressure
  • Collect monthly NQF measure 0059 - Controlling Hemoglobin A1C (HbA1c) levels > 9.0% (poor control) 
  • Workflow analysis and recommendations for changes
  • Use of clinical decision support, evidence-based protocols
  • Patient engagement
  • Creating useful tools and resources for your patients and staff
  • Sharing best practices and lessons learned

Information is organized in the three main topic areas: (1) Hypertension (NQF 0018), Diabetes (NQF 0059), and (3) Prediabetes.
