The Centers for Medicare & Medicaid Services (CMS) Innovation Center announced that it will be funding Practice Transformation Networks (PTN) to provide on-the-ground technical assistance to help primary and specialty care practices achieve the Triple Aim (better health, better care, at lower cost) and thrive in the pay-for-value environment.
The West Virginia Medical Institute (WVMI) & Quality Insights and the Pittsburgh Regional Health Initiative (PRHI) are part of a consortium of quality improvement groups that will be collaborating to provide PTN assistance, if funding is awarded. HealthInsight, a well-respected, non-profit, quality improvement organization, will serve as the project's prime contractor at the multi-state level.
If awarded the contract, this four year outreach initiative allow Quality Insights of Pennsylvania (and PA REACH) the opportunity to continue helping your organization utilize your EHR more efficiently so that providers and can build upon the positive impact these systems can provide, and in turn, continue to elevate the quality of patient care.
The best part about this initiative is that our services will be fully subsidized by the grant at NO COST to providers.
