Quality Insights of Pennsylvania is seeking providers who are willing to test eCQMs for possible inclusion in the Centers for Medicare & Medicaid Services (CMS) EHR Incentive Program. CMS has contracted with Mathematica Policy Research to develop, electronically specify and test eCQMs. Quality Insights successfully tested several measures in the past year as a partner with Mathematica.

Measure to be tested: Percentage of patients 75 years or older at the start of the measurement period with documentation in the EHR at least once of (1) results from a standardized cognitive impairment, OR (2) a patient or informant interview.

 

Criteria for consideration as a test site:

• Outpatient or ambulatory care setting
• Site has at least one provider who serves at least 30 patients over the age of 75
• Physician or non-physician staff member knowledgeable about point-of-care process and familiar with the scope of the EHR.
• Data from a cognitive impairment assessment using a standardized structured tool( Mini-Cog, GPCOG,MMSE for example )  records the assessment results in a structured field  OR
• Conducts structured interviews to assess cognitive impairment and records the results or that interview occurred OR
• Conducts "informant"  interviews to assess cognitive impairment and records the results or that interview occurred OR
• Data must be captured in a structured, extractable field in the EHR (12 months of data)

Benefits to Participants:

• Opportunity to directly help refine and validate eCQMs to be considered in the MU program
• Practices will receive assistance from Quality Insights in determining data availability, defining current workflow and establishing the EHR design and the feasibility of implementing the measure.
• Selected practices will be eligible for an incentive payment (up to) $4,000.
